Our pharmacists at Atoms Pharmacy can provide Medication Therapy Management (MTM) to help patients get the most benefit from their medications, and eventually, optimize therapeutic outcomes. MTM can help detect and prevent possible costly medication problems in the future.
In our MTM program, our pharmacists can:
- Review a patient’s prescribed medications and properly assess and manage any OTC and herbal products that the patient is taking to detect any medication problems.
- Provide in-depth medication-related education to patient and their family to ensure the proper usage of medication.
- Collaborate with other healthcare providers, including the patient and his or her physician, to achieve the best results of medication therapy.
